In the case of a near miss, first –aid case ( FAC), restricted work case ( RWC), lost time injury (LTI) and a fatality (FAL), a committee is set-up to investigate and analyze each and every issue, full report transcribed. The object of the committee is to use each issue as a tool and advise all line crews of each and every case, as the intelligent gained from the investigation can be the preventive action necessary to avoid similar incidents.
